2. A description of the infringing material
3. The URL of the infringing material
4. A statement that you have a good faith belief that the material infringes on the rights of the copyright owner
5. A statement that, under penalty of perjury, you are the copyright holder or are legally authorized to act on behalf of the copyright owner
6. A statement that all information is the message is accurate

eHo.st is not responsible for payments you should receive via Google Adsense or the performance of your Adsense account. We cannot be held responsible for the content which is provided by users on pages displaying your ads.
